Overview
Overview The Program Facilitator is responsible for addictions treatment programming for Indigenous families and counseling services, crisis intervention and case management for individual, group and families. Services will focus on guiding clients to solve problems and make positive changes in their life. Program Facilitators are also responsible for addressing the day-to-day concerns, issues and needs of clients.
Responsibilities
- Cofacilitate the successful delivery of the Addiction Treatment Program (twelve-block modules) and the development and delivery of content that will meet the needs, wants and levels of Indigenous families
- Demonstrate diversity awareness
- Assess the success of the current program through on-going client checks, post evaluations and revise the program as needed
- Works with the immediate supervisor and team members to create and maintain a care plan for clients that addresses the prevention and treatment of substance use and concurrent disorders
- Prepares and discusses the client’s treatment plan with the team as necessary
- Provides regular reports on the development progress of clients and provides recommendations to the immediate supervisor including discharge plans
- Support client inclusion in community support services including language, and gender-based supports (sharing circles, men group etc.)
- Facilitates completion of the daily program schedule including physical activities and program sessions
- Provides counselling sessions to clients and families
- Conducts daily sharing circles. Documents all important information in client files (either written or electronic)
- Accompanies and transports clients to appointments (i.e., medical, social, legal))
- Manages crisis and behavioral response with a strength-based response and an addiction’s complexity lens
- Attend regular communication meetings (Huddles)
- Attend and contribute to program crossover meetings on Mondays
- Attend Case Conference meetings and present family progress notes
- Documents clients progress in all development sessions
- Participates in improving CJAY clinical program
- Plan evening extra-curricular activities that interest clients and enhance skills (such as cultural ceremonies, beading, sewing, music lessons, and wood working etc.)
- Assist in developing, implementing, and modifying treatment plans and strategies to achieve the best outcome
- Encourage clients to participate and involve them in the plan to return to their community (aftercare plan)
- Ensure that clients receive the necessary and appropriate encouragement to continue in their healing journey, personal growth, and development
- Promote healthy group interaction between clients
- Motivates clients to work together in group activities
- Conducts daily medication administration requirements
- Attend and participate in all team and staff meetings as requested
- Complete all paperwork as needed such as bi-weekly time sheets and leave requests
- Complete documentation of incident reports, medical reports, and case notes as required
- Together with the treatment team maintains client database and client file
- Completes the daily log for every shift
- Completes client daily progress note per shift before leaving shift
- Maintains and records necessary certifications, licenses and mandatory training requirements in a learning portfolio and submits at annual performance review
- Seeks opportunities and attends training for professional growth and competency building
Qualifications
- Diploma in Social Work, Human Services OR Mental Health and Addictions Counseling
- Ability to function independently, to be flexible and adapt to change
- High level of professionalism and able to maintain confidentiality at all times
- Ability to communicate clearly
- Ability to work under pressure and in crisis situations
- Must maintain a healthy lifestyle recognizing they are viewed as a role model in the Community
- Class 4 Drivers license
- Fluency in an Innu or Indigenous language is an asset
